Welcome to Pharmacy on the Bosque, LLC. We are a licensed non-sterile compounding pharmacy operating in accordance with federal and the state of New Mexico regulations. These and conditions (“Terms”) outlines the agreement between you and Pharmacy on the Bosque regarding using our services, including compounded medication dispensing, consultation, any retail services, and deliveries. By using our services through our website, apps, and contact forms, you agree to these Terms. If you do not agree with any part of Terms, you may not access or use our website, apps, and any contact forms.

Prescription Requirements

• All compounded medications require a valid prescription from a licensed healthcare provider in the state of New Mexico.

• Pharmacy on the Bosque verify all prescriptions and may contact your provider for clarification.

• Compounded medications are personalized for you and are not FDA-approved.

• However, they are prepared in accordance with FDA guidelines and United States Pharmacopeia (USP) and the National Formulary(NF).

 

Patient Responsibility and Informed Consent

• By using our service, you acknowledge that compounded preparations are customized for you and may differ in strength, dosage for, or ingredients from commercially available products.

• By using our service, you agree to consult with your healthcare provider regarding the use, benefits and potential risks associated with compounded preparation prior to use for your therapy.

• By receiving our compounded preparation personalized for you, you consent to our compounding services as described by your prescriber. Privacy and Confidentiality (HIPAA)

• Pharmacy on the Bosque protects your health information in accordance with the Health Insurance Portability and Accountability Act (HIPAA).

• Your information shall be only used or disclosed for treatment, payment, healthcare operation, or other permitted uses and disclosures stated at the Privacy policies unless otherwise authorized by you in writing.

• The information provided by Pharmacy on the Bosque on the website, printed materials, or any other communication channels is for informational purpose only. It is not intended as medical advice. It should not be used as a substitute for healthcare provider’s medical judgement, diagnosis, or treatment.

• Compounded preparations are prepared specifically for individual patients based on a valid prescription from a licensed prescriber. The safety and efficacy of compounded medications are not reviewed by FDA and the results may vary.

Contact Form Usage

 

• Using our contact forms, you agree to provide Pharmacy on the Bosque with accurate and complete information.

• Using our contact forms, you agree not to submit any confidential or sensitive personal information not limited to social security numbers, financial information, or medical information.

Payment and Insurance

• Compounded preparations are often not covered by insurance. Pharmacy on the Bosque will not accept any insurance plans. You are responsible for full payment at the begging of service, such as drop-off unless otherwise arranged.

• Pharmacy on the Bosque will provide receipts and documents for reimbursement from your insurance carrier upon request.

• Pharmacy on the Bosque will provide in depth consultation with the cost.

• Pharmacy on the Bosque will not accept cash as payment option. Payment methods accepted include:​​

      2.  Bank cards

Shipping and Delivery.

• Pharmacy on the Bosque only offers shipping services for non-controlled substances compounded preparations in accordance with federal and the state of New Mexico regulation. Some medications may require refrigeration or signature upon delivery.

• Pharmacy on the Bosque will not responsible for delays caused by carriers or failure to retrieve deliveries promptly.

• If there is any damage or lost in shipment, please notify Pharmacy on the Bosque within 48 hours.

Returns and Refunds.

• Due to the pharmacy regulation, all sales are final. Pharmacy on the Bosque does not accept returns or offer refunds unless the compounded preparation is defective or an error was made on our part unless there is any recalled ingredient.

 

Adverse Reactions and Reporting

• If you experience any adverse reaction, please contact your healthcare provider

• immediately and follow their advice.

• You may also report adverse events to Pharmacy on the Bosque and/or the State of New Mexico

        and/or the FDA via MedWatch:

Use of website/app and contact form.

• You must be at least 18 years old to use Pharmacy on the Bosque website/app and contact forms.

• You agree to use Pharmacy on the Bosque website/app and contact forms only for and in accordance with these Terms.

• You agree not to use Pharmacy on the Bosque website/app and contact forms in any way that could interfere or disable the website/app and contact forms.

• You agree not to use any unauthorized tools to access the information from Pharmacy on the Bosque website/app and contact forms.

 

Liability Limitation

• Pharmacy on the Bosque is not liable for any misuse, misadministration, or unauthorized sharing of compounded preparations.

• Use of any compounded preparations should strictly follow the prescribing provider’s instructions.

 

Intellectual Property Rights

• All content on this website, including not limited to text, graphics, logos, icons, images, audio and video clips, downloads and software, is the property of Pharmacy on the Bosque or its content suppliers is protected by applicable copyright, and intellectual property laws.

• You may view, download, and print materials from this website for your personal, non-commercial use only, if you do not modify the materials and retain all copyright and proprietary notice. Any other use including reproduction, modification, distribution, republication or display of the content on this site without prior written permission form Pharmacy on the Bosque, is strictly prohibited.

 

Changes to Terms.

• These Terms are subjected to be updated periodically. Pharmacy on the Bosque will inform patients with any changes via email, or in-store notice.

• Continued use of services Pharmacy on the Bosque provides implies the agreement with the revised Terms.
